William Bloat is a 53 year old male with a BMI of 30 which classifies him as obese (WHO, 2014) and is diagnosed with type II diabetes mellitus. William leads a sedentary lifestyle and works as a crane operator. The highest therapeutic priority for this patient is the small wound on his foot. Delayed healing is likely to be associated with diabetes as wound repair is dysregulated, characterized by reduced collagen synthesis and deposition, reduced resistance to wound breakdown, and impaired leukocyte function (Kaviani et al, 2011, Houreld & Abrahamse, 2010). This is a priority as it prevents William from participating in physical activity, increasing the risk of disease and disability (WHO, 2014). By increasing physical activity the patient will be able to eliminate or reduce his comorbidities, lowering his BMI to an acceptable range and resolving his diabetes. William is currently taking amitriptyline, a tricyclic antidepressant with low-dose analgesic effects. Side effects from this medication are minimal, but may include dizziness, drowsiness, dry mouth, nausea, and constipation (Pain Concern, 2013). William can control his condition through the use of electrophysical devices and an exercise program, which coincide with the provision of chronic pain. relief and healing of wounds. As a result, William will be able to participate in more activities and will allow him to work without pain. According to Colberg et al. (2010), an exercise program consists of resistance exercises performed at moderate to vigorous intensity, using five to ten major muscle groups at least two to three days per week.
